  • Want to hear about your concerns, compliments and complaints,
  • Seek to resolve complaints as soon as possible.

How you can give us feedback:

  • Complete an anonymous Quick Feedback Form available below or in every service location
  • Speak directly to your worker or their Service Manager
  • You can also provide feedback using downloading and completing a compliments and complaints form

If you do not feel confident approaching SMART about a complaint or feel you need help in putting your comments forward, you can ask to speak to your local advocacy service.
